How it works
Three steps to your first visit
Step 1: Complete the forms online
Registration, consent and history take about 10 minutes. Have your insurance card handy.
Step 2: Call or text to schedule
Call or text (770) 615-7000. Tell us your child’s age and whether it is a check-up, a sick visit or a transfer of care.
Step 3: Come to the first visit
Bring your insurance card, ID, immunization and medical records. Plan 40 minutes; we do not rush first visits.

What to bring
- Insurance card and a photo ID
- Immunization records (or the name of the previous practice)
- Prior medical records, or a signed release so we can request them
- A list of current medications and allergies
- School or sports forms that need completing
- Your questions — write them down

Questions new families ask
Do you accept my insurance?
We accept most major plans. Call the front desk with your plan name and we will confirm before your visit.
Can I transfer my child’s care mid-treatment?
Yes. Sign a records release at the first visit and we will request the full chart. Ongoing prescriptions are reviewed and continued at the first visit.
Do you see children with special healthcare needs?
Yes. As a Patient-Centered Medical Home we coordinate with specialists, therapists and schools; care coordination visits are part of what we do.
