Confidential visits from 12, college health checklists, and care through age 21.

Which school forms can you complete?
Fulton County Schools forms, including the Georgia certificate of immunization (Form 3231) and the ear, eye and dental form (3300), sports physicals on the GHSA form, and camp and daycare forms, all at the well visit. We send immunization certificates through GRITS.
Do you work with specialists at Emory Johns Creek?
Yes, and with Children’s Healthcare of Atlanta and Northside. Every referral is tracked until the appointment happens and the note comes back; see our closed-loop referrals.
Do you accept Gwinnett or Forsyth Medicaid plans?
Georgia Medicaid plans are statewide, so the county does not matter. We participate with Amerigroup, CareSource and Peach State, PeachCare for Kids and straight Medicaid, and we are accepting new Medicaid patients.
