Notice of Privacy Practices

This notice describes how medical information about your child may be used and disclosed and how you can get access to this information. Please review it carefully. The full Notice of Privacy Practices is available at the front desk and in the patient portal, and you receive a copy at your first visit.

How we use and share health information

• Treatment: with the providers, specialists, therapists, pharmacies and schools involved in your child’s care, with your permission where the law requires it
• Payment: with your insurance plan or programs such as Medicaid and PeachCare for Kids to bill for care
• Health care operations: quality improvement, training, and the patient-centered medical home program
• As required by law: public health reporting such as immunization registries (GRITS), reportable diseases, suspected abuse or neglect, and court orders

Your rights

• See and get a copy of your child’s record, usually through the patient portal
• Ask us to correct information you believe is wrong or incomplete
• Ask for confidential communication, for example by text or portal message only
• Ask us to limit what we share, and receive a list of certain disclosures we have made
• Get a paper copy of this notice at any time, and file a complaint without any effect on your care

Minors, teens and consent

Georgia law lets teens consent to certain care on their own, and from age 18 the patient controls the record. We explain what this means at adolescent visits and help families set up portal proxy access.
