Two counties, one office
Gwinnett or Forsyth side, the paperwork is the same
Most of Suwanee is in Gwinnett County, and part of it is in Forsyth. Children on the Gwinnett side attend Gwinnett County Public Schools, such as North Gwinnett, Peachtree Ridge and Collins Hill High. On the Forsyth side they attend Forsyth County Schools, such as Lambert High. The Georgia forms are the same for both: we complete Form 3231 (certificate of immunization), Form 3300 (ear, eye and dental) and GHSA sports physicals at the well visit. Georgia Medicaid plans are statewide, so the county does not change your coverage here.

Each child’s care is led by her or by Dr. Adam Ayer, DNP, and the whole team works from one shared chart. Dr. Viswanadham is fluent in Spanish and sees families in Spanish without an interpreter.
Babies born at Northside Hospital Gwinnett, Emory Johns Creek or Northside Forsyth are seen here two to three days after discharge. Expecting parents can book a free prenatal visit first.

How long is the drive from Suwanee?
