Allied and Coordinated Care
As an NCQA-recognized Patient-Centered Medical Home, Dare2Care Pediatrics coordinates care with specialists, therapists, schools and community programs so families with complex or chronic needs have one team that keeps track of everything.
About allied and coordinated care
Some children need more than one clinic: a therapist, a specialist at Children’s Healthcare of Atlanta, a school plan, a community program. Our medical home model means we stay in the middle of it.
How we coordinate
- Referrals with a clear question and a shared record
- Follow-up after specialist visits so the plan actually happens
- School letters, 504 plan and IEP support
- Connections to Babies Can’t Wait, WIC, Reach Out and Read and other Georgia programs
- Care plans for chronic conditions such as asthma, ADHD, diabetes and prematurity
When to come in
- Your child sees more than one specialist
- A school plan or accommodation letter is needed
- You are new to the area and need to rebuild a care team
- A hospital stay just ended
What to expect at the visit
A care-coordination visit reviews every provider, medication and plan your child has, then sets who does what. Bring records and contact details for everyone involved.
Need help outside office hours?
When the office is closed, call the main number, (770) 615-7000. After hours the call goes to our after-hours line for urgent concerns. Use the patient portal for anything that can wait until the next business day. In an emergency, call 911.

What is a Patient-Centered Medical Home?
A recognition from the National Committee for Quality Assurance for practices that organize care around the patient: same-day access, care coordination, evidence-based care and tracking outcomes. It means your pediatrician is the quarterback, not just a stop on the way.
