School Physicals
School physicals cover Georgia Form 3300 (vision, hearing, dental and nutrition screening) and Form 3231 (certificate of immunization) required for daycare, kindergarten, 7th grade and new students, completed during a well visit.
About school physicals
Georgia requires Form 3300 and Form 3231 for children entering a Georgia school or daycare for the first time, and updated immunization certificates at 7th grade. We complete both at the well visit.
What is required
- Form 3300: ear, eye, dental and nutrition screening, within 12 months of enrollment
- Form 3231: certificate of immunization, updated when vaccines are due
- 7th grade: Tdap and meningococcal vaccines documented
- 11th grade: second meningococcal dose documented
When to come in
- Your child is starting daycare, pre-K, kindergarten or a new Georgia school
- Your child is entering 7th or 11th grade
- The school has asked for an updated certificate
What to expect at the visit
If your child has had a well visit within the last year we can often complete the forms from the record. Otherwise we book a well visit and complete them then.
Need help outside office hours?
When the office is closed, call the main number and follow the prompts to reach the on-call provider for urgent concerns. Use the patient portal for anything that can wait until the next business day. In an emergency, call 911.

Can you complete forms without a visit?
If the required screenings were done at a recent well visit, yes. Call the office with the form and we will let you know.
