Sports Physicals

Sports physicals complete the Georgia High School Association and club forms, check heart, lungs, joints and concussion history, and are best done as part of the yearly well visit so nothing is missed and nothing is billed twice.

About sports physicals

A sports physical is a real exam, not a signature. We look for the things that matter on the field: heart murmurs and family history of sudden cardiac death, asthma, prior concussions, joint stability and, for girls, menstrual health and bone density risk.

Tips

  • Book before the season; August fills up fast
  • Bring the school or league form with the parent section completed
  • Combine it with the yearly well visit when possible

When to come in

  • A new season or a new sport is starting
  • Your athlete had a concussion, injury or fainting episode
  • The form is due

What to expect at the visit

A focused exam plus the form, usually 20 minutes. We complete the form on the spot.

Need help outside office hours?

When the office is closed, call the main number and follow the prompts to reach the on-call provider for urgent concerns. Use the patient portal for anything that can wait until the next business day. In an emergency, call 911.
